Healthy Roster

What is Healthy Roster?
- North Catholic’s Athletic Department uses Healthy Roster, an electronic medical records system, to collect all athletic physical forms (PIAA and AHN) as it concerns to participation in extracurricular athletic activities. This platform is also used as a means of communication between the Certified Athletic Trainer and parent.
 
What are the mandatory forms (documents) that need to be filled out online via Healthy Roster?
- The following forms (documents) must be filled out online and signed electronically:
 
  • AHN – Authorization for Release of Protected Info
  • AHN – Consent to Treat
  • PIAA CIPPE - Sections 1
  • PIAA CIPPE – Section 2 - 5
  • PIAA CIPPE - Section 6 (Health History)
  • PIAA CIPPE - Section 7 (Physician’s Clearance)
 
Can I still turn in paper copies of the mandatory forms (documents) to the Athletic Office?
- No. All paperwork must be submitted online via Healthy Roster.

How do I create a Healthy Roster account for my student-athlete?
- You must fill out your child’s information online. Click here to get this process started. You will receive an invitation within 3-5 business days from Healthy Roster to access their account.

I have more than one child participating in athletics, how do I add an additional child into Healthy Roster?
- You must fill out the information of each additional child at HealthyRoster.NorthCatholic.org. You can use the same email address so that all of your athletes fall under the same username.

When can I start filling out the forms? Do they need completed every year?
- Forms are considered valid from June 1st – May 31st of each year. Parents can begin filling out their child’s forms on or after June 1 of the upcoming school year. This process will need to be completed every school year.

My child’s physical was completed prior to June 1st, can I use this physical for the upcoming school year?
- No. The PIAA requires that Section 7, the physicians form, must be dated on or after June 1st in order to be valid.

Do these forms (documents) need to be filled out prior to voluntary, off-season workouts?
- No. All paperwork is due prior to the start of the first official day of practice.

If I have heath care insurance other than Highmark, do I need to complete the Allegheny Health Network – Authorization for Release of Protected Info and Allegheny Health Network – Consent to Treat documents?
- Yes. These forms are necessary for our Certified Athletic Trainers to provide treatment and care to all student-athletes at North Catholic High School.
 
 

Baseline Concussion Testing

When does my child need to complete a baseline concussion test?
- Each student-athlete is required to complete a baseline during their freshman and junior year.

How do I register for a baseline appointment?
 - You will now have two options for accessing a baseline test.  As in previous years, you can schedule an assessment through the link below.  New this year, you can choose to have your child tested during one of his/her off-season workouts. We will be coordinating with each sport and reaching out through the coach and boosters to alert athletes of these opportunities.  Click here to schedule.

Where is the test held?
- Baselines are performed at North Catholic High School. Use the main entrance of the building and report to the main office at your scheduled appointment time.

Do I need to accompany my child to their appointment?
- No. Parents do not need to accompany the student-athlete.

How long will the test take?
- The test takes about 20 minutes.

Does my child need to bring anything with them to complete the test?
 - Yes.  Sway testing is performed using a smart phone.  Please have your child bring their phone with them fully charged.  If your child does not have a smart phone, accommodations can be made.  

My child took a baseline outside of North Catholic, can I use these results instead?
- Yes. You must provide documentation or results or the access code to the results to meet this requirement. The results must be less than 2 years old in order to be considered valid.
